Overview
We are seeking a dedicated Part-Time Social Worker to join our team and make a meaningful difference in the lives of our residents. This role offers an opportunity to provide compassionate social services, support mental health and behavioral needs, and collaborate with interdisciplinary teams to enhance quality of life for seniors and individuals with disabilities.

PRINCIPLE DUTIES:

  1. Assist with completion of Social Service assessments.
  2. Coordinate care with social work job-share partner to provide continuity of care, consistent communication, and smooth transitions for residents and families
  3. Coordinate discharge planning, including referrals to appropriate resources with social work team, family and interdisciplinary team.

TITLE: General Maintenance

QUALIFICATIONS

Education

  • Bachelor’s degree required.

Licensure

  • Current, active, and unencumbered Nursing Home Administrator (LNHA) license required in the State of Ohio, or ability to obtain licensure as required by state regulations.

Experience

  • Minimum of three (3) years of supervisory or management experience in a hospital, long-term care, senior living, or related healthcare setting required.

The Clinical Manager – Memory Care is responsible for the clinical oversight and daily nursing operations of the Memory Care program, including Assisted Living and a 16-bed licensed nursing unit that will be specializing in dementia care. Essential Job Duties

  1. Assists residents with dressing, grooming, eating, bathing, positioning, turning, toileting and exercising.
  2. Transfer, transport, and escort residents as needed.
  3. Answers signal lights promptly.
  4. Changes bed linens, makes beds and keeps rooms clean and orderly.

Qualifications:

  1. Successful completion of a State approved geriatric nursing assistant training program or willing to complete the Nurse Aide training course.
  2. Ability to understand and follow instructions in English, communicate effectively, and perform simple arithmetic.
  3. Ability to document care given.
