GENERAL SUMMARY:

The General Maintenance person is responsible for maintaining all aspects of the physical plant. Conducting under supervisor direction, preventive maintenance, routine servicing, seasonal repairs and changes, building equipment and plumbing repairs.

PRINCIPLE DUTIES:

Essential Job Duties:

Performs general maintenance duties, such as fixing wall covers, floor covers and some furniture and fixtures, light plumbing work, minor electrical repair and appliance repairs.

Performs painting and refurbishing projects, as directed by supervisor.

Performs routine preventive maintenance, as directed by supervisor.

Performs minor building and equipment repairs.

Accomplishes all work in the order of priority set by supervisor.

Assists, during winter, with snow removal efforts.

QUALIFICATIONS:

High school education. Technical Schooling and/or background preferred. Ability to understand and follow instructions in English, communicate effectively, and perform simple arithmetic.

One year’s maintenance experience necessary.

The Charge Nurse is an RN/LPN who is responsible for the overall nursing care/service on his/her shift, including the accurate and timely administration of medications, treatments, documentation and emergency calls, as indicated.

Essential Job Duties:

  1. Supervises direct care of residents and nursing personnel on his/her shift.
  2. Administers direct care as necessary.
  3. Gives and receives shift reports.
  4. Rounds twice a shift to assure proper care is rendered in a safe and clean environment.
  5. Keeps physicians and families aware of changes in resident’s condition.
  6. Assists in developing comprehensive care plans.

Other Duties:

  1. Maintains the self respect, personal dignity, and physical safety of each resident.
  2. Maintains accurate documentation of all care rendered.
  3. Responds to resident emergency call system.

Other Duties (cont.):

  1. Interfaces with other disciplines to assure continuity of care and services.
  2. Accompanies physicians when making rounds.
  3. Encourages restorative care in order to obtain maximum resident independence.
  4. Participates in family/resident counseling as necessary.
  5. Assists with the orientation and evaluation of nursing personnel.
  6. Attends in-service training and education sessions, as assigned.

KNOWLEDGE AND SKILL REQUIREMENTS:

RN/LPN currently licensed in the State.  At least 6 months nursing experience in long term care setting preferred.

PRINCIPLE DUTIES:

1. Collects, cleans and redistributes the Community laundry.

2. Ensures that all Health Center laundry is properly sorted, washed and dried according to State and Federal regulations.

3. Ensures that the laundry is maintained in a clean, orderly and sanitary condition.

TITLE: MDS Nurse

EXEMPT STATUS: Non-Exempt

REPORTS TO: Director of Nursing or MDS Coordinator

GENERAL SUMMARY:

The MDS Nurse is an RN who is responsible for the overall supervision, coordination, maintenance and timely completion of the MDS, care plans and medical records for residents.

PRINCIPLE DUTIES:

Essential Functions:

  1. Coordinates, conducts, and completes the MDS, resident care plans and conferences on the computerized system.
  2. Assures accuracy and completeness of entire medical record.
  3. Assures and monitors that MDSs are completed within HCFA and Medicare guidelines.
  4. Ensures MDS electronic submission occurs in accordance with State regulations.
  5. Participates in family/resident care plan conferences.
  6. Assures effective communication with other shifts and departments as it relates to medical record documentation.

Other Duties:

  1. Encourages rehabilitative techniques in order to restore residents’ independence.
  2. Assists DON in developing and implementing medical records policies and procedures.

QUALIFICATIONS:

RN is currently licensed by State of Ohio. Current CPR certification. In- depth experience and knowledge working with MDS computer software. Experience with the MDS transmittal process and understanding of regulations impacting the MDS. Supervisory experience preferred.

PHYSICAL REQUIREMENTS/WORKING CONDITIONS:

Frequent standing, stooping, bending, stretching, squatting; must be able to stand/walk for up to 70% of the work day; must be able to lift, transfer, turn and position residents weighing up to 125 lbs. unassisted, or with the assistance of one for a resident weighing over 125 lbs. May be subjected to offensive odors and combative behavior. Must be able to lift and carry up to 20 lbs. frequently, and 50 lbs. occasionally. May be exposed to communicable diseases including the HIV and HBV viruses. Must be able to work paying close attention to detail with frequent interruptions.
